Somewhere between a chemistry demonstration and a wellness ritual, methylene blue has become one of the most visible substances in the biohacking world. Influencers post videos of themselves dropping deep-blue liquid onto their tongues, staining them a cartoonish indigo, and describe it as “brain fuel” — a nootropic that sharpens focus, lifts mood, boosts cellular energy, and slows aging. The trend has been amplified by high-profile figures and, unusually for a supplement fad, has drawn comment from the upper reaches of federal health leadership (NPR).
What is genuinely unusual about methylene blue is that its mechanism is not made up. It really does interact with mitochondria in a way that could, in principle, matter for the brain. But that kernel of real biology has been inflated into a set of human claims the evidence does not support — and the compound carries real dangers that the tongue-staining videos never mention.
What Methylene Blue Actually Is
Methylene blue is a synthetic dye first produced in 1876 for the textile industry. Chemically, its defining trick is that it readily swaps electrons with other molecules — behaving, as University of South Carolina pharmacologist Lorne Hofseth describes it, like “a tiny battery charger” (USC, 2025). That electron-shuttling property is the basis for both its legitimate medical uses and its speculative ones.
Its established, FDA-approved use is narrow and specific: treating methemoglobinemia, a rare condition in which hemoglobin can no longer carry oxygen. Methylene blue restores function by donating an electron (Harvard Health). It also has recognized off-label hospital roles — in certain poisonings, in vasoplegia/septic shock, and as a surgical dye to mark tissue. In every one of these settings it is a pharmaceutical-grade drug, dosed by clinicians, for defined indications. None of them is “take a few drops each morning to think better.”
The Mitochondrial Story — and Where It Stops
The nootropic pitch rests on a real observation: methylene blue can cross into the brain and support mitochondria, the cell's energy factories, potentially helping them generate ATP more efficiently. In rodents and cell cultures, this translates into a genuinely impressive list of effects — improved learning and memory, protection of neurons in Alzheimer's-like models, and reduced damage after simulated brain injury or ischemic stroke.
But this is exactly where the marketing quietly substitutes animal data for human proof. As Hofseth notes, most of the encouraging findings come from rats and dishes, and no research has tested whether methylene blue protects human brains from traumatic injury or stroke. In people, the picture is thin and unconvincing: a 2023 review of methylene blue in Alzheimer's clinical trials found results “mixed and not conclusive.” A small 26-person study found a single low dose boosted memory by roughly 7% and increased brain activity on imaging — but the follow-up work found altered brain connectivity without any measurable improvement in thinking skills. Human studies in pain and neuropsychiatric conditions have likewise been small and inconclusive.
The upshot from the pharmacology is blunt: dosing is unsettled, and “much larger, longer trials are needed to know if it truly works, what the right dose is and how safe it is over time.” That is the language of an open research question, not a validated cognitive enhancer.

The Risks the Videos Skip
Methylene blue is not an inert curiosity, and the casual dosing promoted online collides with a serious pharmacological reality: it is a potent monoamine oxidase inhibitor. That means it blocks an enzyme that breaks down serotonin, and combined with the many people taking SSRIs, SNRIs, or other serotonergic medications, it can trigger serotonin syndrome — agitation, confusion, high fever, rapid heart rate, muscle rigidity, and, in severe cases, seizures or death. This is not a fringe worry: it is the subject of a boxed-style warning in the drug's FDA prescribing information, which explicitly cautions against concomitant use with serotonergic drugs (FDA prescribing information).
The hazards do not end there. In people with G6PD deficiency — a common enzyme disorder many don't know they have — methylene blue can cause red blood cells to break down. High doses can raise blood pressure and cause cardiac problems, and it is considered unsafe in pregnancy and breastfeeding (Medscape). Layered on top is a quality problem: much of the methylene blue sold to consumers is not pharmaceutical grade. Industrial or “chemical grade” dye can contain heavy-metal contaminants, and the dose in a dropper bottle is far less controlled than an intravenous ampule administered in a hospital.

What the Evidence Actually Shows
Methylene blue has a real mitochondrial mechanism and a legitimate medical identity — as a hospital drug for methemoglobinemia, not a daily brain supplement. The cognitive, mood, energy, and anti-aging claims driving the trend rest on rodent and cell-culture data plus a couple of tiny, inconclusive human studies, one of which found brain-imaging changes but no improvement in actual thinking. Meanwhile the compound carries genuine risks — a serotonin-syndrome interaction with common antidepressants, danger in G6PD deficiency, and contaminant concerns in non-pharmaceutical product. The mechanism is real; the human benefit is not established; the risks are.
The Bottom Line
Methylene blue is a fascinating molecule with real medical uses and a mechanism intriguing enough to justify continued research into brain aging and injury. But “worth studying” is not “proven to make healthy people sharper,” and the leap the marketing makes — from rat mitochondria to a daily human nootropic that also fights aging — is unsupported by the human evidence. The pharmacologist's own summary is the right one: the claims are alluring, the supporting evidence is scant, and there are serious risks to using it outside medical supervision.
For anyone taking a serotonergic antidepressant, the calculus is simpler still: combining it with methylene blue can be dangerous, and no unproven cognitive edge is worth serotonin syndrome. This is a hospital drug that wandered into the supplement aisle, and it should be treated with the caution a drug deserves.

The mitochondrial mechanism is real and the compound has genuine clinical uses, but the nootropic and anti-aging claims fail on human evidence: only tiny, mixed studies exist, and the most-cited one showed no actual improvement in thinking.
